meets the Level 2 live scan background check requirement in the
state of Florida. Fingerprints are electronically submitted to
FDLE, processed with the FBI and results are delivered to the
state agency requesting the background check.
Required to complete a Level 2 background
check that includes a photo? Some Florida state agencies, such
as the Department of Health (DOH) and the Agency for Health Care
Administration (AHCA) now require a photo with each background
check. FastFingerprints has been certified to submit these background
checks required by AHCA and DOH in Florida, and your photo will
be captured at the time you are fingerprinted.
As of April 1, 2013, visit the Bethel
Road location in Columbus, OH to be fingerprinted for AHCA, DOH
or other Florida purposes. Walk-ins for Florida FDLE background
checks will be welcomed at this location, Mon-Fri 8am - 5pm.
Please be sure to bring a government-issued photo identification
and the appropriate ORI number.

offer fingerprint cards to those who seek licensing outside of
Florida or Ohio. At our locations in Independence, West Chester,
Bethel Road, and Maumee/Toledo, fingerprints can be captured
electronically and printed onto 2 fingerprint cards, which you
can then send to the applicable licensing board in the desired

Effective January 1, 2013, Home
Health Agencies (HHAs) must conduct criminal background checks
on
applicants before hiring, and on all employees, at least once
every five years. HHAs must initially check current employees
hired prior to January 1, 2008 no later than thirty days after
the anniversary of the employees date of hire, and employees
hired on or after January 1, 2008 no later than thirty days after
the fifth anniversary of the date of hire.
